Reading Nutrition Labels Without Being Fooled
Nutrition labels are a public health tool that the food industry has learned to game. Understanding what to look for, and what to ignore, changes how you shop.

The nutrition facts panel was introduced in 1990 as a public health intervention. The goal was to give consumers the information they needed to make informed food choices. In the three decades since, the food industry has become extraordinarily skilled at presenting information on labels in ways that create a misleading impression of healthfulness while remaining technically accurate. Understanding how to read a label correctly, and which claims mean nothing, is a practical clinical skill.
Start with the Serving Size
The first number on any nutrition label and the most important one to check is the serving size. Every other number on the label applies to that serving size, not to the entire package. Food manufacturers set serving sizes, and they have a long history of setting them unrealistically small to make calorie and sugar counts appear more favorable.
A bottle of juice labeled 250 calories may contain 2.5 servings if the manufacturer has defined a serving as 200ml of a 500ml bottle. A bag of chips with 150 calories per serving may contain 3.5 servings. The relevant question is not what the label says per serving but what you will actually consume in a sitting, multiplied by the serving count.
The FDA updated serving size regulations in 2016 to better reflect what people actually eat, and the changes are being gradually implemented. But serving size manipulation remains a common tactic, particularly for beverages and snack foods.
Calories
Total calories per serving, once you have accounted for realistic portion size, is the most direct measure of energy density. For weight management, total caloric intake relative to expenditure is the primary determinant. However, calories alone do not tell you about food quality, macronutrient composition, or how satiating the food will be.
A useful secondary metric is caloric density: calories per gram. Foods below 1.5 calories per gram, including most vegetables, fruits, and legumes, can be consumed in substantial volume without high caloric load. Foods above 4 calories per gram, including most ultra-processed snacks, provide high calories in small physical quantities, making portion control difficult.

Trans fat, generated through partial hydrogenation of vegetable oils, significantly raises LDL and lowers HDL. The FDA effectively banned partially hydrogenated oils in the US food supply in 2018, but foods produced before the deadline may still be in distribution, and small amounts can still appear in products under the rounding threshold of 0.5g per serving, displayed as 0g. "Partially hydrogenated oil" anywhere in the ingredients list means trans fat is present regardless of what the label says.
Sodium
Dietary sodium intake is strongly linked to blood pressure, and elevated blood pressure is the leading modifiable cardiovascular risk factor globally. The American Heart Association recommends no more than 2,300 mg of sodium per day, with an ideal target of 1,500 mg for people with hypertension.
The average American consumes approximately 3,400 mg of sodium daily, and roughly 70% of that comes from processed and packaged foods rather than the salt shaker. Single-serving processed foods can contain 800 to 1,200 mg of sodium — more than half the daily recommended limit. Checking sodium per serving, and extrapolating to realistic portions, is particularly important for people managing blood pressure.
Total Carbohydrates, Fiber, and Sugar
Total carbohydrates include all carbohydrate forms: starch, fiber, and sugar. Breaking this down is essential. Dietary fiber reduces net glucose impact and has independent benefits for cardiovascular risk and bowel health. A food with 30g total carbohydrates and 8g of fiber has a meaningfully different glycemic effect than one with 30g total carbohydrates and 1g of fiber.
Total sugars includes both naturally occurring sugars (from fruit or dairy) and added sugars. The distinction matters. Naturally occurring sugars come packaged with fiber and other nutrients that slow absorption. Added sugars are concentrated glucose and fructose with no accompanying nutritional value. The FDA now requires added sugars to be listed separately on the nutrition facts panel, which is genuinely useful. The AHA recommends no more than 25g of added sugar per day for women and 36g for men. Many single processed foods exceed these limits.
Fiber content is one of the most useful numbers on the label. A food with 5g or more of fiber per serving is a good source; 3g or more is a reasonable source. Most people in high-income countries consume roughly half the recommended 25 to 38g of daily fiber. Scanning labels for fiber content is one of the most practical applications of label reading.
Protein
Protein content per serving is straightforward. Most adults need approximately 0.36 to 0.6 grams of protein per pound of body weight daily, with higher amounts for people engaged in regular resistance training or with increased protein requirements from illness or older age. Checking protein content is particularly relevant when evaluating snacks and meal replacements marketed as high-protein.
The Ingredients List
The ingredients list is often more informative than the nutrition facts panel. Ingredients are listed in descending order by weight, meaning the first three to five ingredients constitute the bulk of the food. If the first ingredient is a refined grain, sugar, or oil, that is what you are primarily eating.
The length of the ingredients list is a rough proxy for processing level. Whole or minimally processed foods have short, recognizable ingredient lists. Ultra-processed foods have long lists containing ingredients that do not appear in home cooking: emulsifiers such as carrageenan and polysorbate 80, artificial colorings, flavor enhancers, and various stabilizers and preservatives. These are not necessarily dangerous at the quantities consumed in individual servings, but their presence indicates a highly processed product.
Sugar has over 60 names in ingredient lists, including high fructose corn syrup, dextrose, maltose, cane juice, agave nectar, and many others. Manufacturers sometimes list multiple sugar forms to push each one lower in the ingredients order, creating the impression that sugar is a minor ingredient when the combined sugar content is substantial. Scanning for multiple sugar synonyms in the first five ingredients is a useful habit.
Front-of-Package Claims
Front-of-package health claims are marketing, not nutrition information. Several warrant particular skepticism. "Natural" has no regulatory definition in the US and means nothing. "Made with whole grains" requires only that some whole grain is present — the first ingredient could still be refined flour. "Low fat" often correlates with higher sugar content, added to compensate for lost flavor. "Reduced sodium" means 25% less sodium than the original product, which may still be a very high absolute amount. "No added sugar" does not mean low sugar if the food contains naturally occurring sugars in concentrated form, as in many dried fruits and juices.
"Heart healthy" claims require meeting specific FDA criteria related to total fat, saturated fat, cholesterol, and sodium, which rules out many processed foods but does not guarantee a nutritionally excellent product. "Gluten-free" indicates the absence of gluten-containing grains and is medically relevant for people with celiac disease or gluten sensitivity. For everyone else, it conveys nothing about the overall nutritional quality of the food.
A Practical Approach
Rather than analyzing every number on every label, a few habits cover most situations. Check serving size first and multiply by realistic portions. Look at added sugar and fiber, which together give a strong signal about food quality. Check sodium for packaged foods if you are managing blood pressure. Read the first five ingredients. Ignore front-of-package claims almost entirely.
For most whole foods, fresh produce, plain meats, fish, legumes, and eggs, labels are minimal or nonexistent. The foods that require the most careful label scrutiny are, as a category, the foods worth consuming least.

Fortification can make a meaningful contribution to nutrient intake, particularly for iron, B vitamins, vitamin D, and calcium, but it does not compensate for a food's overall nutritional profile.
